Posterior Tibial Tendon Dysfunction (PTTD) is an overstretching of the Tibialis Posterior tendon in the foot. The cause is usually long term overuse of the tendon leading to inflammation, pain and degeneration.

The Tibialis Posterior muscle/tendon is crucial to maintaining the arch of the foot. Therefore when it is has been strained over time and it is no longer working properly it can cause flattening of the foot (“flat foot” or “adult-acquired flat foot”). This can lead to heel pain, arch pain, plantar fasciitis &/or heel spurs.

It usually presents as pain on the inside of the arch, foot and ankle and is more common in individuals over 40 years of age.
